New Patients
For those coming to our practice for the first time, we encourage you to register on our patient portal. You will need the security code provided to you when you made your appointment. Please call our office if you do not have one. Once on the portal complete the “demographic” and “forms” tabs and click “submit” to send the forms to us. Should you have any questions, please feel free to contact us.
You may also download the forms below, complete and bring with you to your appointment. There will be a delay at check in if you have not completed the forms.
What to bring to your first appointment:
- Insurance Card
- Photo ID
- List of Medications
- Power Of Attorney paperwork (if applicable)
- Completed New Patient Paperwork (Hard copy or filled out through Patient Portal)
- All minors (any patient under the age of 18) must be accompanied by a parent or legal guardian to first appointment.
